PATH Plaza is a mobile application that helps clients of the PATH program stay connected with their case-management team — viewing appointments, tracking housing milestones, receiving announcements, and messaging staff. This policy explains what information the app handles and how it is protected.
Who is responsible for your information
The PATH program and its client data are administered by the Tehama Continuum of Care, which funds PATH and manages the underlying client records within its Homeless Management Information System (HMIS). The Tehama Continuum of Care is the party responsible for decisions about how client information is collected and used.
The PATH Plaza mobile app is developed and maintained by Waypoint Nexus LLC on behalf of the Tehama Continuum of Care. Waypoint Nexus LLC operates the app and its supporting technical services under the direction of the Tehama Continuum of Care.
Information we collect
PATH Plaza collects only the information needed to verify your identity and provide case-management services through the app:

PATH Plaza does not collect your device's location, contacts, photos, or advertising identifiers, and the app does not contain advertising.
How your information is used
- To verify your identity when you sign in.
- To display your appointments, housing-milestone roadmap, and program announcements.
- To enable secure messaging between you and your case-management team.
- To send notifications related to your case, such as new messages.
Your information is used only to provide these services. It is not sold, rented, or used for advertising.
How your information is stored and protected
App data is stored using Google Firebase (Cloud Firestore), a secure cloud database service. Information is encrypted in transit between your device and the service using industry-standard HTTPS encryption. Access to client information is restricted to authorized Tehama Continuum of Care case-management staff through a secure staff portal.
Google, through its Firebase platform, processes and stores this data on our behalf as a service provider. It does not use your information for its own purposes. You can review Google's data practices in the Firebase Privacy and Security documentation.
How your information is shared
Your information is accessible to authorized case-management staff of the Tehama Continuum of Care in order to provide services to you. It is shared with Google Firebase only as the technical service provider that hosts the app's data. We do not sell your information or share it with advertisers or other third parties.
We may disclose information if required to do so by law, or to protect the safety of a client or others, consistent with the Tehama Continuum of Care's obligations.
How long your information is kept
Client information is generally retained for as long as you are a client of the PATH program, and thereafter in accordance with the Tehama Continuum of Care's HMIS record-retention practices, until deletion is requested. Information is not kept longer than needed for the purposes described in this policy or as required by applicable rules governing HMIS data.
Your choices and requesting deletion
Because your client records are managed by the Tehama Continuum of Care within its HMIS, requests to access, correct, or delete your information are handled by the Tehama Continuum of Care.
To request access, correction, or deletion of your information, please contact your case manager or the Tehama Continuum of Care directly. You may also reach the app developer at the contact below, who will direct your request to the appropriate party.
Children's privacy
PATH Plaza is intended for adult clients (18 and older) of the PATH program. It is not designed for or directed at children, and we do not knowingly collect personal information from children under 18. If you believe a child's information has been provided to us, please contact us so it can be removed.
Changes to this policy
We may update this Privacy Policy from time to time. When we do, we will revise the "Last updated" date above. Significant changes may also be communicated through the app or by the Tehama Continuum of Care.
